CH(F)IRP stands for the Confidential Human Factors Incident Reporting Programme. This is a scheme that allows all civilian aircrew and air traffic control personnel to report their own errors in complete confidence to the RAF Institute of Aviation Medicine. The programme was initiated and sponsored by the Civil Aviation Authority — the CAA. The key point here is that it's confidential: you can report a mistake you made without fear of disciplinary action, and the data is used to improve safety across the industry.
Next, Ciliary Muscles. These are the muscles inside your eye that push and pull on the lens to achieve final focusing. When you look from something far away to something close up, these muscles contract or relax to change the shape of the lens. Now, Circadian Rhythms. Many physiological processes in your body show regular rhythmic fluctuations — they go up and down in a predictable cycle. The most common rhythms in humans and other organisms have a periodicity of about 24 hours — that's where the name comes from: circa meaning "about" and dies meaning "day" in Latin. Circadian Dysrhythmia is simply the disruption of those circadian rhythms. You'll hear this called "jet lag" in everyday language — it's what happens when your internal clock is out of sync with the local time zone.
Co-action means working in parallel toward a common goal. It's about multiple people or systems operating alongside each other, all aiming for the same outcome.
Now, the Cochlea. This is the part of the inner ear that deals with hearing. Here's how it works: vibrations in the air — sounds — reach your eardrum and make it vibrate. That vibration is passed across the middle ear by a series of small bones to the fluid-filled cochlea. Inside the cochlea, there's a sensitive membrane that responds to those vibrations and generates nerve impulses. Your brain then interprets those impulses as sounds.

Next, Cones. These are light-sensitive cells located on the retina at the back of your eye. They are sensitive to colour. When light hits them, they convert that light into nerve impulses that travel up the optic nerve to your brain, where the visual picture is built up. So cones are responsible for your colour vision and fine detail.
Finally, Confirmation Bias. This is a concept in decision-making. Once you've made a decision, there is a natural tendency to... and the excerpt cuts off there, but I can tell you that confirmation bias is the tendency to look for or interpret information in a way that confirms your pre-existing beliefs or decisions, while ignoring evidence that contradicts them. In aviation, this can be dangerous — for example, if you've decided the weather is fine, you might overlook signs that it's actually deteriorating.
